Lymphoedema Management of Physiotherapy
Physiotherapy Management of Lymphoedema
Physiotherapist Gillian Buckley has more than 20 years’ experience in managing lymphoedema and is an APA titled Lymphoedema Physiotherapist and an ALA Lymphoedema Therapist. Gillian leads a team of three lymphoedema physiotherapists at Platinum Physio: herself, Carlie Dean and Dani Feil.
Early access to lymphoedema physiotherapy treatment can prevent the more serious disabling aspects of lymphoedema. New research has shown that regular monitoring and early detection may actually prevent the onset of lymphoedema after breast cancer treatment**.
Lymphoedema is caused by damage to the lymphatic system which results in swelling of body tissue. This swelling in the body is due to the build-up of fluid in soft body tissues when the lymph system is damaged or blocked. If untreated, permanent tissue changes can occur. Frequently occurring in the arms and legs, lymphoedema can also be present in the trunky, groin and head and neck regions. This swelling can cause pain and discomfort, reduced function, impaired mobility and recurrent infection.
Lymphoedema may arise because the lymphatic vessels or nodes have been damaged or were not formed correctly. Primary lymphoedema occurs as a result of a congenital condition that affects how the lymph vessels where formed. Secondary lymphoedema can result from cancer or cancer treatments, or can be as a result of trauma, infection, venous disease or obesity.
Physiotherapy treatment for lymphoedema includes lymphoedema exercise programs, manual lymphatic drainage, compression therapy and low level laser therapy. An important aspect of lymphoedema treatment is education about preventing lymphoedema progression and advice about self management and home programs.
Reducing the risk of lymphoedema involves three important components:
- Screening for lymphoedema ideally starts before cancer treatment and continues following surgery and treatment
- Assessment including Bioimpedance Spectroscopy (SOZO) and LymphScanner (PWC) which can measure and detect early lymphoedema
- Advice regarding risk reduction strategies to prevent lymphoedema
**Ridner SH, et al. A Randomized Trial Evaluating Bioimpedance Spectroscopy Versus Tape Measurement for the Prevention of Lymphedema Following Treatment for Breast Cancer: Interim Analysis. Ann Surg Oncol 2019; https://doi.org/10.1245/s10434-019-07344-5.
Research shows that physiotherapist-supervised exercise programs during and after cancer treatment significantly reduce the risk of developing lymphoedema.
